Prescribing considerations
- Confirm MET exon 14 skipping using a validated assay before treatment; tissue testing is more sensitive, while plasma testing may be used when biopsy is unavailable.
- Initiation and supervision require a clinician experienced in anticancer therapy.
- Review pre-existing lung or liver disease and pregnancy potential.
- Severe renal or hepatic impairment has not been studied.
- The product contains lactose and is unsuitable for certain rare hereditary disorders of galactose metabolism.
Contraindications and cautions
- Hypersensitivity to tepotinib or any excipient.
Monitoring
- Assess promptly for new or worsening respiratory symptoms; withhold while suspected interstitial lung disease is investigated and discontinue permanently if confirmed.
- Check liver enzymes and bilirubin before treatment, frequently during early treatment and regularly thereafter.
- Monitor oedema, weight or functional effects of fluid retention, albumin and treatment tolerability.
- Interpret creatinine-based renal estimates cautiously because OCT2 and MATE inhibition can reduce tubular creatinine secretion; investigate alternative evidence of renal impairment when needed.
- Review interacting P-glycoprotein, BCRP, OCT and MATE substrates.
Clinical pharmacology
Tepotinib inhibits MET, including MET exon 14 skipping variants, and suppresses MET-dependent downstream signalling. Food increases systemic exposure. It is highly protein-bound, metabolism is not its main elimination route, and most recovered material is excreted in faeces.
Formulation and product differences
- The product is a lactose-containing film-coated tablet.
- Tablets may be swallowed whole or dispersed in still water; they must not be crushed or chewed.
- A prepared dispersion can be administered through a suitable nasogastric tube using the product-specific instructions.
