tea tree oil: clinical use

Prescribing considerations

• Traditional-use registration: clinical efficacy has not been established through the evidence normally required for conventional medicines.
• Exclude significant infection, severe acne or a fungal infection requiring definitive treatment.
• Avoid oral administration and inhalation; diluted oromucosal use must not be swallowed.
• Oxidation increases sensitisation potential, making storage and expiry checks clinically relevant.

Contraindications and cautions

• Hypersensitivity to tea tree oil
• Hypersensitivity to colophony
• Use in children under 12 years is not recommended because adequate data are lacking
• Avoid use in the eyes or ears

Monitoring

• No routine laboratory monitoring is specified.
• Monitor for irritation, contact dermatitis or burn-like reactions.
• Reassess worsening lesions, fever, pus, spreading inflammation or failure to improve.
• After ingestion, assess for central nervous system depression and muscle weakness; paediatric ingestion requires emergency hospital management.

Clinical pharmacology

Tea tree oil is a mixture of monoterpenes, with terpinen-4-ol a major constituent. Experimental antimicrobial activity is associated mainly with disruption of microbial membrane integrity, permeability and membrane-linked respiration. Product-specific human pharmacodynamic and pharmacokinetic studies are not provided.

Formulation and product differences

• The selected product is a clear, colourless to pale-yellow cutaneous solution and concentrate for oromucosal solution.
• It contains tea tree oil without excipients.
• The amber glass container has a child-resistant closure and incorporated dropper.
• After opening, storage away from light and heat with the cap tightly closed limits oxidation and sensitisation risk.
