Prescribing considerations
- Confirm that the formulation is licensed and appropriate for the indication and site of bowel disease.
- Assess infection history, respiratory disease, allergy or asthma, hepatic and renal function, blood disorders, G6PD deficiency and pregnancy plans.
- Ensure adequate hydration and counsel about early symptoms of blood, liver, renal, pulmonary, skin or systemic hypersensitivity toxicity.
- Sulfasalazine may interfere with some laboratory assays, including urinary normetanephrine and certain ultraviolet-absorbance methods.
Contraindications and cautions
- Hypersensitivity to sulfasalazine, its metabolites, sulfonamides, salicylates or relevant excipients
- Porphyria
- Children younger than two years
Monitoring
- Before treatment: full blood count with differential and platelets, liver tests, albumin and renal function; assess additional risks individually.
- After starting or changing treatment: monitor blood count, liver tests, albumin and renal function regularly, with more frequent testing in people at higher risk.
- Once stable: continue periodic laboratory monitoring and respond to trends as well as absolute values.
- Stop and investigate promptly if blood dyscrasia, hepatic injury, renal deterioration, severe rash or other systemic toxicity is suspected.
Clinical pharmacology
Most oral sulfasalazine reaches the colon, where bacteria cleave its azo bond to sulfapyridine and mesalazine. Sulfapyridine is substantially absorbed and contributes to systemic effects and many adverse reactions; mesalazine acts mainly within the colonic mucosa. Acetylator status can influence sulfapyridine exposure and tolerability.
Formulation and product differences
- Standard tablets and oral suspension are licensed for ulcerative colitis and active Crohn’s disease.
- Gastro-resistant tablets delay disintegration beyond the stomach, must not be crushed or broken, and are additionally licensed for rheumatoid arthritis.
- The oral suspension is an option when swallowing tablets is difficult and should be taken with food.
- Suppositories are intended for disease affecting the rectum and act predominantly locally, with lower systemic uptake than oral formulations.

sulfasalazine interactions
Tell the prescriber or pharmacist about all medicines, supplements and herbal products. Important examples include:
Digoxin
Oral sulfasalazine can reduce digoxin absorption and may produce inadequate blood concentrations.
Azathioprine or mercaptopurine
The combination can increase bone-marrow suppression and low white-cell counts, requiring careful monitoring.
Methotrexate
Combined treatment may increase gastrointestinal adverse effects, particularly nausea.
Glucose-lowering medicines
Sulfonamide-related effects may increase the risk of low blood glucose, so closer monitoring may be needed.
