Prescribing considerations
- Treatment should be supervised by a clinician experienced in lysosomal acid lipase deficiency, metabolic disorders or chronic liver disease.
- Ensure staff and facilities can recognise and manage anaphylaxis and severe infusion reactions.
- Consider sodium exposure in patients following a controlled-sodium diet.
- Record the product name and batch number to maintain biological-medicine traceability.
Contraindications and cautions
- Life-threatening hypersensitivity to sebelipase alfa when rechallenge has been unsuccessful.
- Life-threatening hypersensitivity to egg or any excipient.
Monitoring
- Observe closely for hypersensitivity during and after initial administration and whenever clinically indicated.
- Monitor clinical and biochemical response, including growth, liver tests, lipid profile, organ size and relevant gastrointestinal features.
- Test for anti-drug antibodies after severe infusion reactions or an unexplained lack or loss of effect.
- Be aware that transient, usually asymptomatic increases in circulating cholesterol and triglycerides may occur after treatment starts.
Clinical pharmacology
Sebelipase alfa is recombinant human lysosomal acid lipase. Glycan-mediated receptor binding enables cellular uptake into lysosomes, where it hydrolyses cholesteryl esters and triglycerides.
Formulation and product differences
- The selected product is a single-use concentrate requiring aseptic dilution with sodium chloride infusion solution.
- It is produced using recombinant technology in egg white and may contain traces of egg proteins.
- Excipients include human serum albumin and sodium compounds.
- Unopened vials require refrigerated, light-protected storage and must not be frozen or shaken.
sebelipase alfa interactions
Formal interaction studies have not been performed. As a recombinant human protein, sebelipase alfa is considered unlikely to cause cytochrome P450-mediated interactions.
