Prescribing considerations
- Initiation and supervision should be by a physician experienced in diagnosing and treating non-segmental vitiligo.
- Confirm non-segmental disease with facial involvement and licensed age eligibility.
- Review previous herpes zoster, phototherapy and non-melanoma skin cancer risk.
- Avoid co-application with other vitiligo treatments on the same area; combination with narrow-band UVB is not established.
- Systemic exposure is limited but may rise with damaged skin or excessive application.
Contraindications and cautions
- Hypersensitivity to ruxolitinib or any excipient.
- Pregnancy.
- Breastfeeding.
- Avoid in end-stage renal disease because safety data are lacking.
Monitoring
- Assess repigmentation and continuing clinical value periodically.
- Check for application-site acne, irritation, allergic reactions and shingles.
- Perform periodic skin examination, particularly where non-melanoma skin cancer risk factors exist.
- Routine laboratory monitoring is not specified for the selected topical product.
Clinical pharmacology
Ruxolitinib selectively inhibits JAK1 and JAK2, reducing interferon-gamma-dependent signalling and chemokines involved in immune-mediated melanocyte destruction. Systemic exposure from topical use is limited; absorbed ruxolitinib is metabolised mainly by CYP3A4 and to a lesser extent CYP2C9.
Formulation and product differences
- The selected UK product is a white to off-white cutaneous cream supplied in a tube.
- It contains propylene glycol, fatty alcohols, parabens, butylated hydroxytoluene and polysorbate, which may cause irritation or allergic skin reactions.
- Topical ruxolitinib is not interchangeable with oral ruxolitinib products.

Ruxolitinib interactions
Systemic interaction risk is considered low after topical use, but same-site combinations require care.
Other topical vitiligo treatments
Do not apply them together on the same skin area because the combination has not been evaluated.

Do not layer them directly over ruxolitinib; follow the separation instructions in the product leaflet.
Narrow-band ultraviolet B phototherapy
The safety and effectiveness of combining it with ruxolitinib cream are not established; specialist review is needed.
