Prescribing considerations

• Initiate under a clinician experienced in managing polycythaemia vera.
• Consider whether a more rapidly acting cytoreductive approach is needed when prompt blood-count control is clinically important.
• Assess thyroid, psychiatric, cardiovascular, hepatic, renal, autoimmune and transplant history.
• Record product name and batch number for biological-medicine traceability.
• Review concurrent CYP1A2/CYP2D6 substrates, sedatives and myelosuppressive treatment.

Contraindications and cautions

• Hypersensitivity to the active substance or excipients
• Uncontrolled pre-existing thyroid disease
• Current or previous severe psychiatric disorder, particularly severe depression or suicidal behaviour
• Severe cardiovascular disease, or recent stroke or myocardial infarction
• History or presence of autoimmune disease
• Immunosuppressed transplant recipient
• Concomitant telbivudine
• Decompensated cirrhosis
• End-stage renal disease

Monitoring

• Regular full blood count, including haematocrit, leucocytes and platelets
• Liver enzymes and hepatic function
• Renal function
• Psychiatric symptoms, including depression and suicidality
• Thyroid function when clinically indicated
• Eye assessment before and during treatment, particularly with diabetes, hypertension or retinal disease; arrange immediate examination for visual symptoms
• Cardiovascular, respiratory, skin and autoimmune adverse effects

Clinical pharmacology

A recombinant interferon alfa-2b conjugated to branched methoxypolyethylene glycol. It activates IFNAR-mediated JAK1, TYK2 and STAT signalling, producing antiproliferative and immunomodulatory effects in haematopoietic cells; pegylation provides sustained absorption and slower elimination.

Formulation and product differences

• The selected product is a clear, colourless to pale-yellow solution in an adjustable pre-filled pen.
• It contains benzyl alcohol and polysorbate 80, which may cause allergic reactions, and is essentially sodium-free.
• The pen requires refrigeration, protection from light and a new compatible needle for each injection.
