Prescribing considerations
- Do not use routinely for nocturnal leg cramps; exclude treatable causes and consider non-pharmacological measures first.
- Review previous reactions to quinine, including after tonic water or other quinine-containing drinks.
- Assess cardiac disease, atrioventricular block, electrolyte disturbance and concurrent QT-prolonging medicines.
Contraindications and cautions
- Hypersensitivity to quinine or an excipient.
- Haemolysis or haemoglobinuria.
- Optic neuritis or tinnitus.
- Myasthenia gravis.
- Previous blackwater fever.
Monitoring
- Monitor for early adverse effects and features of cinchonism.
- Ask about bruising or bleeding and investigate suspected blood dyscrasia promptly.
- For cramp treatment, reassess whether meaningful benefit continues.
- Consider ECG and electrolyte assessment where QT-risk factors are present.
Clinical pharmacology
Quinine is a rapidly acting blood schizontocide. It is extensively metabolised in the liver, predominantly through CYP3A4, and mainly excreted in urine.
