Prescribing considerations
- Confirm the indication is symptomatic congestion and screen for cardiovascular, renal, endocrine, ophthalmic and urinary risk factors.
- Review prescribed and non-prescribed medicines for duplicate decongestants and sympathomimetic interactions.
- Consider the recognised potential for misuse, tolerance and toxicity with excessive or continuous use.
- Check formulation excipients where diabetes, allergies, alcohol exposure or hereditary sugar intolerance is relevant.
Contraindications and cautions
- Hypersensitivity to pseudoephedrine or formulation ingredients
- Severe or uncontrolled hypertension and severe acute or chronic kidney disease or renal failure
- Selected SmPCs also contraindicate cardiovascular disease, diabetes mellitus, hyperthyroidism, phaeochromocytoma and closed-angle glaucoma
- Concomitant sympathomimetic decongestants, beta blockers, MAOIs or recent MAOI treatment
- Product-specific paediatric restrictions apply
Monitoring
- Assess blood pressure and pulse when clinically relevant.
- Advise immediate discontinuation and urgent assessment for symptoms suggesting PRES or RCVS.
- Review new palpitations, urinary retention, neuropsychiatric effects, severe skin reactions, sudden visual loss or symptoms of ischaemic colitis.
Clinical pharmacology
An orally active direct and indirect sympathomimetic that produces alpha-adrenergic vasoconstriction and releases stored noradrenaline. It is readily absorbed, undergoes little metabolism and is largely excreted unchanged in urine; urinary pH influences elimination.
Formulation and product differences
- The selected range includes a tablet and two oral liquids with different licensed age restrictions.
- Sudafed Decongestant Liquid contains sucrose, a preservative and colourant that may matter in diabetes, hereditary sugar intolerance or hypersensitivity.
- Care Decongestant Oral Liquid contains ethanol, colourants, hydroxybenzoate preservatives and propylene glycol-containing flavouring.
- Use the product-specific SmPC and leaflet because excipients and restrictions are not interchangeable.

Monoamine oxidase inhibitors, reversible MAO-A inhibitors and moclobemide
Concomitant use, or MAOI use within the previous 14 days, can cause a dangerous hypertensive crisis.
Other sympathomimetic decongestants
Using oral or nasal decongestants together can add cardiovascular and nervous-system effects and should be avoided.
