Prescribing considerations
- Restricted to administration by healthcare professionals using aseptic technique.
- Assess anterior-chamber angle and glaucoma risk before use.

Contraindications and cautions

- Closed-angle glaucoma unless previously treated by iridectomy, or a narrow angle liable to closure after mydriasis.
- Children younger than 12 years; use is not recommended from 12 to 17 years because adequate experience is lacking.
Monitoring
- Confirm the insert remains correctly positioned and is removed after adequate mydriasis.
- Monitor for conjunctival irritation, corneal injury and symptoms of raised intraocular pressure.

Clinical pharmacology
Phenylephrine is a direct alpha-sympathomimetic that contracts the pupillary dilator. Tropicamide is an antimuscarinic that inhibits the iris sphincter and ciliary muscle. Local delivery produces mydriasis with low expected systemic exposure, although systemic adrenergic or anticholinergic effects remain possible.
Formulation and product differences
- The selected product is a white to yellowish-white, oblong ophthalmic insert rather than an eye-drop solution.
- Each sterile sachet is accompanied by disposable sterile forceps.
- It is single use and should be used immediately after opening; retained inserts have caused serious local corneal reactions.
