Prescribing considerations
- Confirm the child meets a licensed high-risk category and that palivizumab is the locally selected antibody.
- Consider deferral during moderate or severe acute infection or febrile illness; a mild upper respiratory infection alone does not usually require deferral.
- Ensure immediate treatment for anaphylaxis is available.
- Consider the effect of cardiopulmonary bypass on serum palivizumab concentrations in specialist planning.
- Record the product name and batch number for biological traceability.
Contraindications and cautions
- Hypersensitivity to palivizumab, an excipient or another humanised monoclonal antibody.
- Use caution with thrombocytopenia or coagulation disorders because administration is intramuscular.
Monitoring
- Observe for acute hypersensitivity after administration.
- Assess unexpected fever, rash, apnoea, bruising, petechiae or neurological symptoms.
- Interpret negative immune-based RSV tests or viral cultures cautiously alongside clinical findings.
- Maintain product and batch traceability.
Clinical pharmacology
Palivizumab is a humanised IgG1κ antibody directed against antigenic site II of the RSV fusion protein. It neutralises RSV A and B and inhibits viral fusion, providing passive rather than active immunity.
Formulation and product differences
- The selected UK product is a ready-to-use solution for intramuscular injection.
- It is supplied in single-use vials and contains no preservative.
- The solution should be clear or slightly opalescent; do not dilute, shake or mix it with other medicines.
- Store refrigerated, protect from light and do not freeze.

palivizumab interactions
Formal medicine-interaction studies have not been conducted. Practical considerations include vaccines and RSV diagnostic testing.
Routine childhood and influenza vaccines
Palivizumab is not expected to interfere with vaccine immune responses, and clinical studies did not identify an added adverse-reaction signal with commonly used vaccines.
Bronchodilators and corticosteroids
Concurrent use occurred in clinical studies without an observed increase in adverse reactions, although formal interaction studies are lacking.
RSV antigen tests and viral culture
Palivizumab can cause false-negative results; reverse-transcriptase PCR testing is not expected to be affected.
