Prescribing considerations
- Confirm previous tolerability or responsiveness to paliperidone or risperidone where required by the licence.
- Not appropriate when immediate control of acute agitation or severe psychosis is required.
- Review renal function, cardiovascular and QT risk, seizure history, diabetes risk, thromboembolism risk, Parkinson's disease or dementia with Lewy bodies.
- Consider prolonged release and delayed reversibility of adverse effects before administration.
Contraindications and cautions
- Hypersensitivity to paliperidone, risperidone or a listed excipient.
- The selected product is not recommended in moderate or severe renal impairment.

Clinical pharmacology
A poorly water-soluble ester prodrug that dissolves slowly after intramuscular injection and is hydrolysed to paliperidone. Paliperidone is principally eliminated renally, with limited hepatic metabolism, and antagonises dopamine D2, serotonin 5-HT2 and alpha-adrenergic receptors.
Formulation and product differences
- Paliperidone palmitate brands and longer-acting presentations have product-specific eligibility, administration and switching instructions and should not be treated as directly interchangeable.
- The amount of paliperidone palmitate in the syringe differs from the stated equivalent amount of active paliperidone.

Medicines that prolong the QT interval
May add to the risk of abnormal heart rhythm.
Alcohol, opioids, sleeping medicines and other sedatives
May increase sleepiness and impairment.
Levodopa and other dopamine agonists
Paliperidone may reduce their effects, while these medicines may oppose its antipsychotic action.
