orlistat: clinical details
Prescribing considerations
- Confirm that the selected product's licensed BMI and risk-factor criteria are met and use it alongside a nutritionally balanced, mildly reduced-calorie diet.
- Review clinical benefit against the product's stopping criteria.
- Review concurrent medicines, particularly those with narrow therapeutic indices or absorption-dependent efficacy.
- Weight loss can alter requirements for glucose-lowering, antihypertensive and lipid-lowering treatment.
- High dietary fat intake increases gastrointestinal adverse effects.
Contraindications and cautions
- Hypersensitivity to orlistat or an excipient
- Chronic malabsorption syndrome
- Cholestasis
- Breastfeeding
Monitoring
- Weight response and continued clinical benefit
- Glycaemic control and diabetes treatment where relevant
- INR or other coagulation parameters with oral anticoagulants
- Ciclosporin concentrations if unavoidable concurrent use is specialist-managed
- Thyroid control with levothyroxine or iodine salts
- Seizure control with antiepileptic medicines
- Clinical or ECG status with amiodarone
- Nutritional adequacy and possible fat-soluble vitamin deficiency
- Renal symptoms in chronic kidney disease or volume depletion
- Severe or persistent rectal bleeding, hepatic symptoms or pancreatitis symptoms
Clinical pharmacology
Orlistat irreversibly inhibits gastric and pancreatic lipases within the gastrointestinal lumen. Systemic absorption is minimal, and most administered medicine is eliminated in the faeces.
Formulation and product differences
- The selected Xenical product is a prescription-only hard capsule with a gelatin shell.
- The selected capsule is essentially sodium-free; other orlistat products can differ in strength, legal category, licensing and excipients.

orlistat interactions
Orlistat can alter the absorption or clinical control of several medicines and nutrients. Review all prescribed, pharmacy and complementary products before use.
Ciclosporin
May lower ciclosporin concentrations and reduce immunosuppression; the combination is not recommended, and specialist-level monitoring is required if unavoidable.
Warfarin and other oral anticoagulants
Anticoagulant control may change, so coagulation monitoring is required.
Acarbose
Concurrent use should be avoided because interaction studies are unavailable.
