omalizumab: clinical use

Prescribing considerations

• Initiate through a clinician experienced in the relevant specialist condition.
• For allergic asthma and nasal polyps, establish pretreatment total IgE and body weight and confirm that the patient falls within the product's licensed selection parameters.
• Do not use for an acute asthma attack, acute bronchospasm or status asthmaticus.
• Assess previous anaphylaxis, renal or hepatic impairment, autoimmune or immune-complex disease, and risk of helminth infection.
• Record the product name and batch number for biological traceability.
• Avoid inadvertent brand or device switching without prescribing, supply and administration safeguards.

Contraindications and cautions

• Hypersensitivity to omalizumab or any excipient.
• Known allergy to polysorbate 20 in the selected product.

Monitoring

• Supervise initial administration and ensure facilities for treating anaphylaxis are available.
• Educate patients and caregivers to recognise immediate and delayed serious allergic reactions.
• Review clinical response and the continuing need for treatment.
• Remain alert for eosinophilia, vasculitic features or worsening systemic symptoms, particularly when oral corticosteroids are reduced.
• Investigate unexplained bleeding or bruising where clinically indicated.

Clinical pharmacology

Omalizumab is a recombinant humanised IgG1 kappa monoclonal antibody that binds circulating IgE, prevents interaction with the high-affinity IgE receptor and reduces free IgE available to drive allergic inflammation.

Formulation and product differences

• The selected Omlyclo presentation is a single-use pre-filled syringe containing a clear to opalescent solution for subcutaneous injection.
• Omlyclo syringes and pens are manufactured without natural-rubber latex-derived materials.
• Device and packaging colours differ between omalizumab brands and presentations; provide device-specific training after a switch.
• Omlyclo has product-specific room-temperature stability and should otherwise be refrigerated, protected from light and never frozen.
