Prescribing considerations

• Initiate under the supervision of a clinician experienced in haemophilia, with immediate access to treatment for anaphylaxis.
• Individual response varies; assess clinical haemostasis and factor VIII activity rather than relying only on body weight.
• Record the product name and batch number for every administration.
• Consider cardiovascular risk and complications from any central venous access device.

Contraindications and cautions

• Hypersensitivity to octocog alfa or any excipient
• Known allergy to mouse or hamster proteins

Monitoring

• Clinical control of bleeding
• Plasma factor VIII activity, particularly around major surgery
• Factor VIII inhibitors if response is inadequate or bleeding remains uncontrolled
• Signs of hypersensitivity during administration
• Central-line infection or thrombosis where a venous access device is used

Clinical pharmacology

Octocog alfa is recombinant human coagulation factor VIII. After infusion it binds endogenous von Willebrand factor and acts as a cofactor in the intrinsic coagulation pathway, restoring thrombin generation and fibrin-clot formation temporarily.

Formulation and product differences

• The selected product is a powder supplied with sterile water for intravenous injection and a BAXJECT preparation system.
• It is produced in Chinese hamster ovary cells without added exogenous human- or animal-derived protein during manufacture.
• The formulation contains sodium and polysorbate 80 and may contain trace mouse or hamster proteins.
• Once stored at room temperature for the permitted single period, unopened packs must not be returned to refrigerated storage.
