Prescribing considerations
- Reconcile prescribed, pharmacy and non-prescription medicines because oral gel is systemically absorbed and inhibits CYP2C9 and CYP3A4.
- Assess swallowing development and choking risk in infants.
- Ensure carers understand front-of-mouth application in small portions and avoidance of the posterior pharynx.
Contraindications and cautions
- Hypersensitivity to miconazole, related imidazoles or product excipients.
- Infants below the licensed age threshold or anyone without a sufficiently developed swallowing reflex.
- Liver dysfunction.
- Concurrent use with specified CYP3A4 substrates, including certain QT-prolonging medicines, ergot alkaloids, simvastatin, lovastatin, triazolam and oral midazolam.
Monitoring
- If prescription co-administration with an oral anticoagulant is judged necessary, closely monitor anticoagulant effect.
- Consider phenytoin concentration and toxicity monitoring.
- Monitor for hypoglycaemia with sulfonylureas.
Clinical pharmacology
Miconazole inhibits fungal ergosterol synthesis and alters membrane lipid composition. Some oral-gel miconazole is absorbed; CYP2C9 and CYP3A4 inhibition accounts for many interactions.
Formulation and product differences
- The sugar-free pharmacy product specifically contraindicates warfarin; the other selected product describes carefully monitored co-administration where clinically planned.

miconazole interactions
Oral gel is partly absorbed and inhibits CYP2C9 and CYP3A4, so clinically important interactions can occur.
Warfarin and other oral anticoagulants
Miconazole may greatly increase anticoagulation and bleeding risk. The pharmacy product is contraindicated with warfarin; planned prescription use requires careful specialist monitoring.
Simvastatin or lovastatin
Concentrations and toxicity risk may increase; concurrent use is contraindicated.
