Prescribing considerations
- Verify vaccination history, current pregnancy status, immune competence, febrile illness, allergy history and recent blood-product exposure.
- Assess individual and family histories of seizures and anticipate post-vaccination fever.
- For thrombocytopenia or coagulation disorders, use the subcutaneous route and assess bleeding risk.
- Record the product name and batch number for biological traceability.
- Ensure treatment for anaphylaxis is immediately available.
Contraindications and cautions
- Hypersensitivity to a measles-, mumps- or rubella-containing vaccine or an ingredient, including neomycin.
- Pregnancy.
- Acute illness with fever above the product-specified threshold.
- Active untreated tuberculosis.
- Blood dyscrasias, leukaemia, lymphoma or another malignancy affecting haematopoietic or lymphatic systems.
- Current significant immunosuppressive therapy.
- Severe primary or acquired humoral or cellular immunodeficiency.
- Family history of congenital or hereditary immunodeficiency unless immune competence is demonstrated.
Monitoring
- Observe for immediate hypersensitivity after administration.
- Advise about expected fever, rash and local reactions and provide clear escalation advice.
- Consider serological assessment where previous vaccine-associated thrombocytopenia makes further vaccination uncertain.
- Monitor eligible patients with selected immune deficiencies carefully for clinical infection because their immune response may be reduced.
Clinical pharmacology
Live attenuated Enders’ Edmonston measles, Jeryl Lynn mumps and Wistar RA 27/3 rubella strains induce active humoral and cellular immune responses. Pharmacokinetic assessment is not applicable to vaccines.
Formulation and product differences
- MMRVAXPRO is a powder with supplied solvent and is available with solvent in a vial or pre-filled syringe.
- It contains hydrolysed gelatine, traces of neomycin and sorbitol; the measles and mumps viruses are produced in chick embryo cells.
- A gelatine-free MMR product may be available in the UK for people who wish to avoid porcine gelatine.

Tell the vaccination service about medicines, blood products, tests and other vaccines because these can affect safety, timing or the immune response.
Immunoglobulins, blood or plasma products
Antibodies in these products may reduce the vaccine response, so vaccination timing and follow-up must be planned by the clinical team.
Immunosuppressive medicines, including high-dose systemic corticosteroids
These may make a live vaccine unsafe or less effective. Current immunosuppressive treatment is a contraindication except for specified low-dose, topical or replacement corticosteroid use.
Other live-virus vaccines
They may be given together at separate sites or require planned separation; the vaccination service should coordinate them.
