Prescribing considerations
- Initiate under a clinician experienced in fertility treatment and use with follitropin alfa.
- Assess infertility and contraindications to pregnancy; evaluate thyroid dysfunction, adrenocortical deficiency and hyperprolactinaemia.
- Consider OHSS, multiple pregnancy, ectopic pregnancy and thromboembolic risks.
- Use particular caution with porphyria or a family history of porphyria and with current or previous thromboembolic disease or recognised risk factors.
- Record product name and batch number for biological traceability.
Contraindications and cautions
- Hypersensitivity to lutropin alfa or an excipient
- Hypothalamic or pituitary tumour
- Unexplained ovarian enlargement or ovarian cyst not related to polycystic ovarian disease
- Unexplained gynaecological bleeding
- Ovarian, uterine or breast carcinoma
- Primary ovarian failure
- Genital malformation or uterine fibroid incompatible with pregnancy
Monitoring
- Assess follicular development by ultrasound and ovarian oestrogen response.
- Monitor actively for excessive ovarian response and OHSS, including after later fertility-treatment steps.
- Review for symptoms of ovarian torsion, ectopic pregnancy and thromboembolism when clinically indicated.
- Safety, efficacy and pharmacokinetics have not been established in renal or hepatic impairment.
Clinical pharmacology
Recombinant human LH produced using genetically engineered Chinese hamster ovary cells. It supports ovarian steroidogenesis and follicular maturation, complementing FSH activity. Subcutaneous bioavailability is approximately 56%, accumulation is minimal and urinary excretion is low.
Formulation and product differences
- The selected product is a single-use powder-and-solvent vial requiring reconstitution before subcutaneous injection.
- The prepared solution must be used immediately and should not be used if cloudy or particulate.
- Store in the original package, protected from light and below the specified storage limit.
- It may be reconstituted with compatible follitropin alfa powder when the specialist has instructed combined administration.
lutropin alfa interactions
Formal interaction studies have not been performed. Tell the fertility team about all prescribed, non-prescribed and herbal products.
Follitropin alfa
It may be administered alongside lutropin alfa and may be mixed in the same injection when specifically directed; studies found no important change in either medicine's activity or stability.
Other injectable medicines
Do not mix them with lutropin alfa in the same injection because compatibility has not been established.
