Prescribing considerations

• Confirm that an antipruritic and hydrating cream is appropriate rather than treatment for infection or an acute inflammatory lesion.
• Review other topical medicines because urea may enhance their penetration.
• Counsel about irritation on broken or inflamed skin and fire risk from residue on fabrics.
• The selected product is licensed for adults, children and infants older than one month.

Contraindications and cautions

• Hypersensitivity to lauromacrogols, urea or an excipient.
• Acute erythroderma.
• Acute inflammatory, oozing or infected skin lesions.

Monitoring

• Assess symptoms and skin-condition response.
• Check for burning, erythema, worsening pruritus, pustules or hypersensitivity.
• Review persistent or worsening symptoms and reconsider infection or an alternative diagnosis.

Clinical pharmacology

Urea is a humectant that increases water retention in the stratum corneum; little is absorbed topically. Lauromacrogols provide a local antipruritic, anaesthetic-like effect, with no evidence of systemic availability after topical use.

Formulation and product differences

• The selected cream contains liquid paraffin, dimeticone derivatives, glycerol and benzyl alcohol among its excipients.
• It is supplied in tube and pump presentations; not every listed pack size is necessarily marketed.
