Prescribing considerations
- Specialist supervision and immediate access to resuscitation facilities are required.
- Assess current clinical status before infusion; acute illness may increase infusion-reaction risk.
- Use particular caution with severe MPS I-related upper-airway or pulmonary involvement.

Clinical pharmacology
Recombinant human alpha-L-iduronidase is taken into cellular lysosomes, probably through mannose-6-phosphate receptors. As a protein, it is expected to undergo peptide hydrolysis; renal elimination is a minor clearance pathway.
Formulation and product differences
- The product is a single-use intravenous concentrate requiring aseptic dilution with sodium chloride solution.
- Use an infusion set with an appropriate in-line filter.
- Unopened vials require refrigerated storage.
- The formulation contains sodium and polysorbate 80.
