interferon beta-1a: clinical use

Prescribing considerations

• Initiate under a clinician experienced in multiple sclerosis treatment.
• Confirm a licensed relapsing presentation or qualifying high-risk first demyelinating event; exclude alternative diagnoses where relevant.
• Assess depression, suicidal ideation, seizures, cardiac disease, renal or hepatic impairment and myelosuppression.
• Record the product name and batch number for traceability.
• Reassess injection technique and sites when local reactions occur.

Contraindications and cautions

• Hypersensitivity to natural or recombinant interferon beta or any excipient.
• Current severe depression and/or suicidal ideation.

Monitoring

• Disease activity, disability progression and continuing treatment suitability.
• Mood and suicidal ideation.
• Full and differential blood count, platelets, blood chemistry and liver-function tests.
• Renal function, proteinuria, oedema and blood pressure where indicated.
• Features of thrombotic microangiopathy; promptly investigate platelets, LDH, blood film and renal function if suspected.
• Cardiac status in people with angina, heart failure or arrhythmia.
• Injection sites and self-administration technique.

Clinical pharmacology

A recombinant, glycosylated type I interferon that activates cell-surface interferon receptors and induces interferon-responsive gene products. Its exact disease-modifying mechanism remains uncertain.

Formulation and product differences

• Clear, colourless, preservative-free solution in a single-use, spring-powered pre-filled pen for intramuscular injection.
• Refrigerate and protect from light. Do not freeze; room-temperature storage is limited to the stated product conditions.
• Inspect visually and discard if the pen is damaged or the solution contains particles or discolouration.
