Prescribing considerations
- Confirm adult type 2 diabetes and need for both components; they cannot be adjusted independently.
- Assess hypoglycaemia awareness, injection technique, ability to read the pen counter and concurrent glucose-lowering medicines.
- Review renal and hepatic function, previous pancreatitis, thyroid disease, gastrointestinal motility disorders, retinopathy and heart-failure risk.
- Ensure treatment is disclosed before general anaesthesia or deep sedation because delayed gastric emptying may leave residual gastric contents.
- Rapid improvement in glucose control may temporarily worsen diabetic retinopathy.
Contraindications and cautions
- Hypersensitivity to insulin degludec, liraglutide or an excipient.
- Not appropriate for type 1 diabetes or diabetic ketoacidosis.
- Not recommended with inflammatory bowel disease, diabetic gastroparesis or severe hepatic impairment.
- Cannot be recommended in end-stage renal disease; there is no therapeutic experience in NYHA class IV heart failure.
Monitoring
- Blood glucose patterns and HbA1c, with additional checks during illness, treatment changes or altered injection sites.
- Hypoglycaemia and changes in warning symptoms.
- Renal function and hydration if gastrointestinal adverse effects persist.
- Injection sites for lipodystrophy or cutaneous amyloidosis.
- Symptoms of pancreatitis, gallbladder disease, intestinal obstruction or hypersensitivity.
- INR with warfarin or another coumarin anticoagulant.
- Weight, oedema and heart-failure symptoms with pioglitazone.
Clinical pharmacology
Insulin degludec produces sustained basal insulin action through slowly absorbed multi-hexamers. Liraglutide is a long-acting GLP-1 analogue with glucose-dependent insulinotropic and glucagon-lowering effects and a modest delay in gastric emptying. Their complementary actions lower fasting and post-meal glucose.
Formulation and product differences
- Fixed-ratio, clear and colourless solution in a disposable multidose pre-filled pen.
- Both ingredients are delivered together in linked dose steps and cannot be adjusted independently.
- For subcutaneous injection only; it must not be mixed, infused or withdrawn into a syringe.

Medicines and substances that can alter glucose control or create additional risks include:
Sulfonylureas and other glucose-lowering treatments
Can increase hypoglycaemia risk; monitor glucose closely when treatments are combined or changed.
Beta-blockers
May mask warning symptoms of low blood glucose and alter insulin requirements.
Glucocorticoids, thiazide diuretics, thyroid hormones and sympathomimetics
May raise blood glucose and change treatment requirements.
