Prescribing considerations
- Specify and verify the brand, formulation, strength and delivery device.
- Switch products only after individual assessment, device training and a monitoring plan.
- Ensure PumpCart users have a compatible pump, appropriate consumables and an alternative insulin-delivery method.
- Assess vision, dexterity and ability to use the selected device safely.
- Record the product name and batch number for biological-medicine traceability.
Contraindications and cautions
- Hypersensitivity to insulin aspart or a product excipient.
- Do not administer while hypoglycaemia is present or suspected.
- NovoMix 30 must not be given intravenously, intramuscularly or through an insulin pump.
- Use added caution with reduced hypoglycaemia awareness or delayed food absorption; renal, hepatic, endocrine or acute intercurrent illness may alter insulin requirements.
Monitoring
- Monitor glucose closely when initiating, switching products, changing injection sites or during illness, pregnancy or altered activity and food intake.
- Review recurrent hypoglycaemia, nocturnal events and impaired warning awareness.
- Inspect injection and infusion sites for lipodystrophy, cutaneous amyloidosis and local reactions.
- Monitor more closely in renal or hepatic impairment and after pump interruption.
Clinical pharmacology
Insulin aspart is a recombinant analogue in which aspartic acid replaces proline at B28, reducing hexamer formation and accelerating subcutaneous absorption. Receptor activation increases peripheral glucose uptake and suppresses hepatic glucose output. Protamine crystallisation in NovoMix 30 prolongs absorption of its intermediate component.
Formulation and product differences
- Trurapi pre-filled pen: clear rapid-acting solution for subcutaneous injection only; it cannot be diluted, mixed or used in a pump.
- NovoRapid PumpCart: clear rapid-acting solution supplied in a cartridge designed only for specified compatible insulin pumps; it must not be used in a pen.
- NovoMix 30 Penfill: biphasic suspension containing soluble and protamine-crystallised insulin aspart in a 30/70 ratio; it requires resuspension and a compatible reusable pen.
