Prescribing considerations

• Treatment should be initiated and supervised by a clinician experienced in LHON.
• Evidence in hepatic or renal impairment is limited; use cautiously.
• Safety and efficacy in children younger than 12 years have not been established.
• Reddish-brown chromaturia is usually benign but should not obscure haematuria or other urinary pathology.
• The product contains lactose and sunset yellow FCF.

Contraindications and cautions

• Hypersensitivity to idebenone or any excipient.

Monitoring

• Assess vision before treatment and at regular specialist reviews.
• Consider additional clinical or laboratory assessment for hepatic, renal, blood or urinary concerns.
• Report suspected adverse reactions through the MHRA Yellow Card scheme.

Clinical pharmacology

Idebenone is a short-chain benzoquinone antioxidant. It may bypass mitochondrial complex I by transferring electrons to complex III, supporting ATP generation. It is rapidly absorbed with food, extensively metabolised and excreted mainly in urine as metabolites.

Formulation and product differences

• The selected product is an orange, round film-coated tablet that must be swallowed whole with food.
• It contains lactose monohydrate and sunset yellow FCF, which may be clinically relevant in susceptible patients.
