Prescribing considerations
- Confirm vitamin B12 deficiency and its cause where practicable before routine replacement; indiscriminate treatment may obscure folate deficiency.
- Investigate folate metabolism if megaloblastic anaemia does not respond as expected.
- Assess previous serious cobalt reactions individually. Cutaneous cobalt allergy alone is not an absolute contraindication to vitamin B12 replacement.
- Cyanokit supplements rather than replaces airway management, oxygenation, cardiovascular support, seizure treatment and appropriate decontamination.
- Cyanokit can discolour plasma and interfere with biochemical, haematological, coagulation and urine assays; alert the laboratory.
Contraindications and cautions
- Cobalin-H and Neo-Cytamen: hypersensitivity to hydroxocobalamin or formulation ingredients.
- Neo-Cytamen should not be used for megaloblastic anaemia of pregnancy unless vitamin B12 deficiency has been demonstrated.
- Cyanokit lists no absolute contraindications; known hypersensitivity is weighed against the life-threatening emergency.
Monitoring
- For deficiency treatment, monitor clinical response, blood counts and neurological status.
- Monitor potassium during initial correction because hypokalaemia-associated arrhythmias have been reported.
- Monitor platelets early in treatment of megaloblastic anaemia because reactive thrombocytosis can occur.
- After Cyanokit, monitor blood pressure and renal function, and interpret laboratory results with awareness of colour interference.
Clinical pharmacology
Hydroxocobalamin is a vitamin B12 analogue transported in plasma by transcobalamins and retained in body stores. As an antidote, its cobalt centre binds cyanide to form cyanocobalamin, which is excreted in urine.
Formulation and product differences
- Cobalin-H is a ready-to-use intramuscular hydroxocobalamin injection for vitamin B12-related indications.
- Neo-Cytamen is an intramuscular hydroxocobalamin chloride solution for vitamin B12-related indications.
- Cyanokit is a high-concentration powder requiring reconstitution and intravenous infusion. It is licensed for cyanide poisoning and is not interchangeable with vitamin B12 replacement ampoules.

hydroxocobalamin interactions
Hydroxocobalamin has few established medicine interactions. Tell clinicians about all medicines, supplements and herbal remedies.
Chloramphenicol
The blood-cell response to hydroxocobalamin may be reduced.
Oral contraceptives
Blood hydroxocobalamin concentrations may be lower, but treatment effectiveness is not expected to fall. Vomiting or severe diarrhoea can separately reduce pill protection.
Antimetabolites and many antibiotics
They may interfere with microbiological vitamin B12 assays, affecting interpretation rather than hydroxocobalamin’s action.
