Prescribing considerations

• Initiate and supervise treatment through clinicians experienced in haemophilia management.
• Individual response varies; assess clinical effectiveness and factor VIII recovery, particularly around major surgery.
• Record the brand and batch number after every administration for traceability.
• Consider hepatitis A and B vaccination for patients receiving plasma-derived factor VIII regularly or repeatedly.
• Consider cardiovascular risk and complications associated with central venous access devices.

Contraindications and cautions

• Hypersensitivity to human coagulation factor VIII or any product excipient.
• Haemoctin is not indicated for von Willebrand disease because it lacks a pharmacologically effective quantity of von Willebrand factor.

Monitoring

• Clinical control of bleeding and plasma factor VIII activity
• Factor VIII inhibitor testing when recovery is inadequate or bleeding remains uncontrolled
• Potential discrepancies between one-stage and chromogenic assay results
• Hypersensitivity during administration
• Central-line infection, bacteraemia or catheter-site thrombosis where applicable

Clinical pharmacology

Plasma-derived factor VIII binds circulating von Willebrand factor and acts as a cofactor in intrinsic-pathway factor X activation. Replacement temporarily restores deficient procoagulant activity; pharmacokinetics and recovery vary between patients and products.

Formulation and product differences

• Both products are plasma-derived powders supplied with water for injection.
• Optivate contains human von Willebrand factor; Haemoctin does not contain it in pharmacologically effective quantities.
• The products differ in concentration after reconstitution, excipients, sodium content, supplied transfer equipment and permitted handling after preparation.
• Do not assume preparation or administration instructions are interchangeable between brands.
