Prescribing considerations
- Establish the indication, previous vaccination, serology and exposure history where relevant.
- Immune response may be reduced by renal failure, immune suppression, older age, obesity, smoking and some chronic diseases.
- Consider subcutaneous administration only exceptionally for thrombocytopenia or bleeding disorders.
- Record the product name and batch number.
- Ensure immediate recognition and treatment of anaphylaxis are available.
- For very premature infants receiving Engerix B, consider the stated risk of post-immunisation apnoea and respiratory monitoring.
Contraindications and cautions
- Hypersensitivity to the active substance, excipients or relevant manufacturing residues, or hypersensitivity following a previous dose.
- Postpone vaccination during severe febrile illness or significant acute infection; a minor infection alone is not a contraindication.
- The HBVAXPRO 40 micrograms vial stopper contains natural latex, requiring caution in latex-sensitive people.
Monitoring
- Consider anti-HBs testing in dialysis, immune suppression, occupational exposure and other situations where confirmation of response affects management.
- Assess significant exposure promptly because vaccination may form only part of post-exposure management.
- Observe for immediate allergic reactions and protect people who faint from injury.
Clinical pharmacology
Recombinant hepatitis B surface antigen produced in yeast is adsorbed onto an aluminium-containing adjuvant and induces protective anti-HBs antibodies and immunological memory. Pharmacokinetic assessment is not applicable to vaccines.
Formulation and product differences
- Engerix B is an aluminium-hydroxide-adsorbed suspension in a pre-filled syringe, with presentations intended for different age groups.
- HBVAXPRO 40 micrograms is an aluminium hydroxyphosphate sulfate-adsorbed vial presentation licensed specifically for predialysis and dialysis adults.
- HBVAXPRO may contain traces of formaldehyde and potassium thiocyanate; its vial stopper contains dry natural latex rubber. Engerix B pre-filled syringe components use synthetic rubber.
- Neither selected product contains live hepatitis B virus.
