Prescribing considerations
- Confirm that symptoms include both productive cough and upper-airway congestion.
- Check for duplicate cough-and-cold products, sympathomimetics and clinically important antidepressant interactions.
- Seek medical review for persistent or recurrent cough, fever, rash, persistent headache, haemoptysis, excessive secretions or suspected chronic respiratory disease.
- Use caution in moderate-to-severe renal impairment, severe hepatic impairment, urinary difficulty, prostatic enlargement, asthma, chronic bronchitis or emphysema.
Contraindications and cautions
- Hypersensitivity to either active ingredient or an excipient.
- Cardiovascular disease, including hypertension; beta-blocker treatment.
- Other sympathomimetic decongestants.
- Diabetes mellitus, closed-angle glaucoma, hyperthyroidism or phaeochromocytoma.
- Current or recent monoamine oxidase inhibitor use.
- Severe acute or chronic kidney disease or renal failure.
- Children under 12 years.
Monitoring
- Assess blood pressure and cardiovascular risk before recommending pseudoephedrine-containing treatment.
- Advise patients to stop treatment and seek urgent assessment for PRES or RCVS symptoms, sudden visual loss, ischaemic colitis symptoms or severe skin reactions.
- Consider urinary retention, palpitations, agitation, insomnia and worsening hypertension if adverse effects develop.
- Guaifenesin metabolites may interfere with urinary 5-HIAA and VMA testing.
Clinical pharmacology
Pseudoephedrine has direct and indirect sympathomimetic activity, causing vasoconstriction in congested upper-airway mucosa. Guaifenesin is thought to increase the volume and reduce the viscosity of respiratory secretions. Pseudoephedrine is mainly eliminated in urine, and its half-life may be substantially prolonged by renal impairment; guaifenesin undergoes oxidation and demethylation.
Formulation and product differences
- The selected formulation is a clear orange-red oral syrup supplied with a measuring spoon.
