Prescribing considerations
- Confirm iron deficiency with appropriate laboratory tests.
- Verify the injectable iron product and brand because licensed indications and administration instructions differ.
- Assess previous parenteral-iron reactions, allergies, severe asthma, eczema, inflammatory or immune disease, liver dysfunction, infection and pregnancy.
- Administer only where staff and facilities for recognising and treating anaphylaxis are immediately available.
- Avoid paravenous leakage and stop administration immediately if it occurs.
Contraindications and cautions
- Anaemia not caused by iron deficiency.
- Iron overload or impaired iron utilisation, including haemochromatosis or haemosiderosis.
- Hypersensitivity to ferric derisomaltose, the product or its excipients.
- Known serious hypersensitivity to another parenteral iron product.
- Decompensated liver disease.
Monitoring
- Haemoglobin, ferritin and transferrin saturation to assess response and avoid iron accumulation.
- Hypersensitivity during administration and for at least 30 minutes afterwards.
- Cannula or injection site for pain, swelling or extravasation.
- Liver status and infection where relevant; do not use during ongoing bacteraemia.
- Be aware that parenteral iron may transiently distort serum bilirubin and calcium results.
- Monitor the fetus during administration in pregnancy.
Clinical pharmacology
After intravenous administration, the complex is taken up mainly by reticuloendothelial cells in the liver and spleen. Iron is released into physiological transport and storage proteins; renal elimination of complexed iron is negligible.
Formulation and product differences
- Diafer is a single-use, dark-brown solution for intravenous injection and may also be given through the venous limb of a haemodialysis circuit.
- If diluted, Diafer must be mixed only with sterile sodium chloride solution; other medicines must not be added.
- Ferric derisomaltose products are not operationally interchangeable: confirm the brand, licensed indication and product-specific instructions.
