evolocumab: clinical use

Prescribing considerations

• Exclude secondary causes of hyperlipidaemia or mixed dyslipidaemia before initiation.
• Use alongside diet and appropriate background risk-factor management.
• Provide device-specific administration training.
• Use caution in severe hepatic impairment, which has not been studied.
• Assess latex-allergy history because the SureClick needle cover contains a latex derivative.
• Record the product name and batch number for traceability.

Contraindications and cautions

• Hypersensitivity to evolocumab or any excipient.

Monitoring

• Monitor lipid profile and clinical response.
• Consider closer monitoring in moderate hepatic impairment because exposure may be reduced.
• Review injection technique, adherence, injection-site reactions and hypersensitivity symptoms.

Clinical pharmacology

Evolocumab is a recombinant human IgG2 monoclonal antibody that binds circulating PCSK9, prevents LDL-receptor degradation and increases hepatic clearance of LDL cholesterol. It is degraded through immunoglobulin pathways into peptides and amino acids rather than conventional hepatic metabolism.

Formulation and product differences

• Single-use SureClick pre-filled pen for subcutaneous injection.
• The solution should be clear to opalescent, colourless to yellowish and practically free from particles.
• The needle cover contains dry natural rubber derived from latex.
• Once removed from refrigeration, the pen may be kept in its original carton at room temperature up to 25°C and used within one month.
