evinacumab: clinical use

Prescribing considerations

• Confirm homozygous familial hypercholesterolaemia and optimise background LDL-lowering management before initiation.
• Use under a clinician experienced in lipid disorders.
• Administration does not need to be coordinated with lipoprotein apheresis.
• Record the product name and batch number for traceability.
• The proline content may be harmful in hyperprolinaemia type I or II; polysorbate 80 can cause allergic reactions.
• Report suspected adverse reactions through the Yellow Card scheme.

Contraindications and cautions

• Hypersensitivity to evinacumab or any excipient.

Monitoring

• Assess LDL-cholesterol response and continuing clinical need.
• Observe for hypersensitivity and infusion-related reactions; stop and treat serious reactions.
• Monitor tolerability, including dizziness, fatigue and infusion-site symptoms.
• Maintain brand and batch traceability.

Clinical pharmacology

Evinacumab is a recombinant human IgG4 monoclonal antibody that inhibits ANGPTL3. It enhances lipoprotein and endothelial lipase activity, promoting VLDL processing and remnant clearance upstream of LDL formation. It is expected to be broken down into peptides and amino acids rather than metabolised through conventional hepatic enzyme pathways.

Formulation and product differences

• The sterile concentrate for intravenous infusion requires dilution with compatible sodium chloride or glucose solution and must not be shaken.
• Discard cloudy, discoloured or particulate-containing solution and any unused vial contents.
