Prescribing considerations
- Initiate and supervise treatment through a team experienced in multiple myeloma.
- Use only within an appropriate licensed combination and review the product information for every component.
- Give specified premedication and ensure facilities are available to manage infusion-related reactions.
- Account for elotuzumab interference when interpreting serum protein electrophoresis and immunofixation results.
Contraindications and cautions
- Hypersensitivity to elotuzumab or any excipient.
- Contraindications belonging to lenalidomide, pomalidomide or dexamethasone also apply to the relevant combination.
Monitoring
- Observe for infusion-related reactions during administration.
- Monitor for infection, particularly pneumonia.
- Remain alert for second primary malignancies.
- Interpret response testing carefully in IgG kappa myeloma because elotuzumab may create an additional assay peak.
Clinical pharmacology
Elotuzumab is a humanised IgG1 antibody against SLAMF7. It activates natural killer cells and promotes antibody-dependent cellular cytotoxicity and macrophage-mediated phagocytosis. It is expected to undergo catabolic degradation to peptides and amino acids.
Formulation and product differences
- Supplied as a white to off-white powder for concentrate in different single-vial presentations.
- All presentations require reconstitution and dilution before intravenous infusion.
- Presentations differ slightly in vial identification and sodium content but contain the same active ingredient and excipients.

elotuzumab interactions
Dedicated pharmacokinetic interaction studies have not been performed. Requirements for the accompanying treatments remain important.
Lenalidomide
Its contraindications, pregnancy-prevention programme and other safety requirements apply when combined with elotuzumab.
Pomalidomide
Its contraindications, pregnancy-prevention programme and other safety requirements apply when combined with elotuzumab.
CYP enzyme inhibitors or inducers
A clinically meaningful effect on elotuzumab exposure is not expected because monoclonal antibodies are not normally cleared through CYP metabolism.
