Efanesoctocog alfa: clinical use

Prescribing considerations

• Initiate and supervise treatment through a clinician experienced in haemophilia care.
• Not licensed for children younger than 2 years.
• Record the product name and batch number for biological traceability.
• Consider cardiovascular risk and complications associated with central venous access devices.

Contraindications and cautions

• Hypersensitivity to efanesoctocog alfa or any excipient.

Monitoring

• Assess clinical control and investigate factor VIII inhibitors if expected activity is not achieved or bleeding remains uncontrolled.
• Measure factor VIII activity during major surgery or life-threatening bleeding.
• Assay results are reagent-dependent; chromogenic and some one-stage assays may overestimate activity and require product-specific interpretation.

Clinical pharmacology

A recombinant factor VIII–Fc fusion protein incorporating a von Willebrand factor D'D3 domain and XTEN polypeptides. It replaces factor VIII while reducing binding to endogenous von Willebrand factor and using Fc recycling to prolong circulating activity.

Formulation and product differences

• Presentations use a powder vial with solvent in a pre-filled syringe; vial potency varies.
• Use the reconstituted solution immediately and do not mix it with other medicinal products.
• The formulation contains sucrose, calcium chloride dihydrate, histidine, arginine hydrochloride and polysorbate 80.
