Prescribing considerations
- Confirm a licensed indication and assess thromboembolic benefit against bleeding risk.
- Calculate creatinine clearance using Cockcroft–Gault before treatment; reassess if renal function may decline.
- Review age, body weight, gastrointestinal disease, hepatic function, interacting medicines and adherence.
- Plan interruption around invasive procedures according to renal function, bleeding risk and procedural urgency.
- Ensure patients understand bleeding precautions, missed-dose instructions, capsule handling and the need to carry anticoagulant identification.
Contraindications and cautions
- Active clinically significant bleeding or a lesion or condition carrying a significant risk of major bleeding.
- Severe renal impairment in adults; the paediatric renal threshold differs and is formulation-specific.
- Concomitant anticoagulation except in specified switching, catheter or ablation circumstances.
- Hepatic impairment or liver disease expected to affect survival.
- Specified strong P-glycoprotein inhibitors, including systemic ketoconazole, itraconazole, ciclosporin, dronedarone and glecaprevir/pibrentasvir.
- Prosthetic heart valves requiring anticoagulation.
- Hypersensitivity to the active substance or excipients.
Monitoring
- Before treatment: full blood count, liver function, urea and electrolytes, serum creatinine with calculated creatinine clearance, body weight and baseline clotting screen.
- During treatment: periodically review renal and hepatic function, full blood count, adherence, bleeding, thrombosis and interacting medicines; monitor more often with frailty, older age, renal impairment or intercurrent illness.
- Routine INR monitoring is inappropriate. In selected urgent or high-exposure situations, diluted thrombin time, ecarin clotting time or activated partial thromboplastin time may assist interpretation.
- Investigate unexplained falls in haemoglobin, haematocrit or blood pressure for occult bleeding.
Clinical pharmacology
Dabigatran etexilate is an orally absorbed prodrug converted by esterases to dabigatran, a competitive, reversible direct inhibitor of free and fibrin-bound thrombin. It is a P-glycoprotein substrate, is not metabolised through cytochrome P450, and active dabigatran is eliminated mainly unchanged in urine; exposure and half-life rise as renal function declines. Idarucizumab is the specific reversal agent for adults when rapid reversal is required.
Formulation and product differences
- The selected product is a hard capsule that must remain intact because opening it substantially increases bioavailability.
- Capsules can be used only by patients able to swallow them whole. Other age-appropriate presentations include coated granules and powder with solvent for oral solution.
- Different formulations are not directly interchangeable without reviewing the prescription because paediatric selection depends on age, weight and swallowing ability.

Other anticoagulants
Concurrent use is generally contraindicated because bleeding risk increases, except in defined circumstances such as supervised switching or catheter management.
Aspirin, clopidogrel and other antiplatelets
These impair clotting by another mechanism and may increase bleeding risk.
NSAIDs such as ibuprofen or naproxen
They can increase gastrointestinal and other bleeding, particularly with ongoing use.
