Prescribing considerations
- Confirm a clear intention to stop using nicotine-containing products.
- Review tobacco, vaping and nicotine-product use.
- Use caution in ischaemic heart disease, heart failure, hypertension, phaeochromocytoma, peripheral vascular disease, peptic ulcer, gastro-oesophageal reflux, hyperthyroidism, diabetes and schizophrenia.
- Not recommended in renal or hepatic impairment because clinical experience is lacking.
- Not recommended for people under 18 or over 65 because evidence is limited.
- Review psychiatric history and medicines affected by smoking cessation.
Contraindications and cautions
- Hypersensitivity to cytisinicline or an excipient
- Unstable angina
- Recent myocardial infarction
- Clinically significant cardiac arrhythmia
- Recent stroke
- Pregnancy
- Breastfeeding
Monitoring
- Watch for mood, behavioural or psychiatric deterioration during smoking cessation.
- Assess symptoms suggesting excess nicotine effects, including cardiovascular or gastrointestinal effects.
- Review blood pressure and heart rate when clinically indicated, particularly with cardiovascular disease.
- Monitor the effect or concentration of narrow-therapeutic-index medicines affected by smoking cessation.
Clinical pharmacology
Cytisinicline is a plant alkaloid and partial agonist, mainly at alpha-4 beta-2 nicotinic acetylcholine receptors. It competes with nicotine, limits nicotine-driven dopamine-system activation and provides weaker receptor stimulation that reduces withdrawal. It is only slightly metabolised and is predominantly excreted unchanged in urine.
Formulation and product differences
- The product is a round, biconvex white oral tablet.
- Excipients include mannitol, microcrystalline cellulose, magnesium stearate, glycerol dibehenate and hypromellose.

Cytisinicline interactions
Review all medicines and nicotine products before starting. Some important effects arise from stopping smoking rather than cytisinicline itself.
Anti-tuberculosis medicines
Cytisinicline should not be used with these medicines.
Nicotine-containing products, including tobacco
Concurrent use can intensify nicotine-related adverse effects.
Systemic hormonal contraceptives
It is unknown whether effectiveness is reduced, so an additional barrier method is required.
