Prescribing considerations
- Initiate under a physician experienced in haematological disorders.
- Confirm meningococcal vaccination status and provide antibiotic prophylaxis where required by the product information.
- Consider pneumococcal and Haemophilus influenzae type b vaccination according to local guidance.
- Use caution with active systemic infection and provide the patient or caregiver guide and patient card.
- Plan transitions from other C5 inhibitors carefully because immune-complex reactions may occur.
- Record product name and batch number for biological traceability.
Contraindications and cautions
- Hypersensitivity to crovalimab or an excipient
- Unresolved Neisseria meningitidis infection
- Absence of current meningococcal vaccination unless appropriate antibiotic prophylaxis is provided
Monitoring
- Early symptoms of meningococcal and other systemic infections
- Haemolysis control, including relevant clinical features and laboratory markers
- Immune-complex symptoms after switching C5 inhibitors
- Administration-related and hypersensitivity reactions
- Loss of exposure or efficacy potentially caused by anti-drug antibodies
- Serious haemolysis if treatment is discontinued without alternative PNH therapy
- Body weight periodically and when clinically appropriate
Clinical pharmacology
Crovalimab is a recombinant humanised IgG1 monoclonal antibody that binds C5, blocks C5 cleavage and membrane attack complex formation, and undergoes antibody recycling associated with prolonged systemic persistence.
Formulation and product differences
- Single-use solution diluted for intravenous infusion or used undiluted for subcutaneous injection.
- Intravenous preparation and administration require a healthcare professional; subcutaneous administration may be undertaken by a trained patient or caregiver when appropriate.
- Store refrigerated, protect from light and do not freeze; follow product-specific limits for temporary room-temperature storage.

Other C5 inhibitors, including eculizumab and ravulizumab
Switching between C5 inhibitors can form immune complexes and cause temporary Type III immune-complex reactions, requiring planned transition and close monitoring.
Medicines metabolised through cytochrome P450 enzymes
Pharmacokinetic interactions are not expected because monoclonal antibodies are cleared differently from small-molecule medicines.
