Prescribing considerations
- Confirm corrected calcium is not below the laboratory reference range before initiation.
- Not indicated for secondary hyperparathyroidism in chronic kidney disease without dialysis.
- Use caution in seizure disorders, cardiac impairment, QT-prolongation risk and moderate-to-severe hepatic impairment.
- Avoid concurrent etelcalcetide and review medicines that lower calcium or alter CYP3A4, CYP1A2 or CYP2D6 activity.
- Paediatric use requires specialist oversight and reliable adherence to calcium monitoring.
Contraindications and cautions
- Hypersensitivity to cinacalcet or a formulation excipient.
- Hypocalcaemia.
Monitoring
- Corrected serum calcium before treatment, after initiation or treatment changes, and regularly thereafter.
- Parathyroid hormone during treatment for secondary hyperparathyroidism.
- Symptoms of hypocalcaemia and, where relevant, cardiac rhythm or heart-failure deterioration.
- Clinical response and calcium control in parathyroid carcinoma or primary hyperparathyroidism.
- Closer clinical surveillance in moderate-to-severe hepatic impairment.
Clinical pharmacology
Cinacalcet is an allosteric calcimimetic at the calcium-sensing receptor. It lowers PTH and serum calcium, is highly protein bound, is mainly metabolised through CYP3A4 and CYP1A2, and strongly inhibits CYP2D6.
Formulation and product differences
- Film-coated tablets must not be chewed or crushed.
- Granule capsules are opened rather than swallowed; the full contents are given with soft food or liquid.
- Granules have specific enteral-tube compatibility and handling instructions.
- The selected Mimpara granule presentations are listed by eMC as discontinued; current supply should be checked.

Check all prescribed, over-the-counter and complementary products before starting or stopping them.
Etelcalcetide
Do not use concurrently because the combination may cause severe hypocalcaemia.
Other medicines that lower calcium
May add to cinacalcet's effect and require closer calcium monitoring.
Strong CYP3A4 inhibitors or inducers, including azole antifungals, ritonavir and rifampicin
Can alter cinacalcet exposure, so treatment should be reviewed when they are started or stopped.
