Prescribing considerations
- Confirm that symptoms reflect a simple mouth ulcer or denture irritation; assess persistent, atypical or worsening lesions.
- Use cautiously with cardiac or hepatic dysfunction.
- Excessive application or use on traumatised mucosa may increase systemic lidocaine exposure.
- The selected product is not indicated for teething.
Contraindications and cautions
- Hypersensitivity to lidocaine, cetylpyridinium chloride, excipients or other amide-type local anaesthetics
- Age under 5 months
- Known or suspected methaemoglobinaemia
Monitoring
- Review persistent or worsening symptoms and examine for trauma, infection or an ill-fitting denture.
- Watch for hypersensitivity, methaemoglobinaemia and systemic local-anaesthetic toxicity where absorption may be increased.
Clinical pharmacology
Lidocaine reversibly inhibits neuronal membrane depolarisation and sensory impulse transmission. Cetylpyridinium chloride is a quaternary pyridinium antiseptic and cationic surfactant. Lidocaine is absorbed through mucosa and predominantly metabolised by the liver.
Formulation and product differences
- The selected preparation is a clear, banana-flavoured oromucosal gel containing ethanol and an eugenol-containing fragrance; it is essentially sodium-free.

Interactions are considered unlikely with correct local use, but increased absorption or combined local anaesthetic exposure can raise risk.
Cimetidine
May increase the risk of lidocaine toxicity.
Beta blockers, including propranolol and bisoprolol
May increase lidocaine exposure and toxicity risk.
Other local anaesthetics or related antiarrhythmics such as mexiletine
