Prescribing considerations
- Confirm that the cough is acute and productive rather than chronic or unexplained.
- Review asthma, acute bronchospasm and porphyria before recommending the product.
- Check concurrent cough suppressants and multi-ingredient cough or cold remedies.
- The oral solution contains ethanol and sorbitol and is essentially sodium-free.
Contraindications and cautions
- Hypersensitivity to either active ingredient or any excipient.
- The selected product is contraindicated below 6 years and not recommended below 12 years.
Monitoring
- Reassess a cough that persists, recurs or occurs with fever, rash or persistent headache.
- Consider possible interference with urinary 5-HIAA or VMA testing and transient changes in platelet-aggregation test patterns.
- In overdose or prolonged excessive use, monitor for gastrointestinal symptoms; prolonged guaifenesin exposure has been associated with urinary stone formation.
Clinical pharmacology
Guaifenesin is readily absorbed, rapidly oxidised and excreted in urine. It promotes expectoration by reducing sputum viscosity and adhesiveness. Cetylpyridinium chloride is a cationic oral antiseptic with antibacterial activity, particularly against Gram-positive organisms; product-specific pharmacokinetic data are unavailable.
Formulation and product differences
- Clear, pale-straw oral solution supplied in glass or plastic bottles.
- Plastic-bottle packs include a graduated measuring cup that should be rinsed after use.
- Contains glycerol, ethanol, levomenthol, sorbitol, saccharin, sodium cyclamate, povidone and flavourings.
- Store below 25°C. Do not refrigerate or freeze.

Cough suppressants
Do not combine them with this expectorant because suppressing the cough may hinder clearance of loosened mucus.

Guaifenesin may increase its absorption rate, although the clinical importance is unknown.
