Prescribing considerations
- Confirm that symptoms match the multi-symptom indication and consider whether simpler single-ingredient treatment is more appropriate.
- Reconcile non-prescription products to prevent duplication of paracetamol, phenylephrine or other decongestants.
- Use particular caution with severe hepatic or renal impairment, non-cirrhotic alcoholic liver disease, cardiovascular disease, hypertension, hyperthyroidism, diabetes or closed-angle glaucoma.
- Consider ethanol, propylene glycol, sorbitol, maltitol and hereditary fructose intolerance when assessing suitability.
Contraindications and cautions
- Hypersensitivity to an active ingredient or excipient.
- Pregnancy.
- Concomitant MAOI treatment or use within the previous 14 days.
- Concomitant beta-blocker or antihypertensive treatment according to the selected SmPC.
- Prostatic enlargement or prostate-related urinary symptoms.
- Concurrent paracetamol-containing, cold, flu or decongestant products.
Monitoring
- Assess blood pressure, palpitations and urinary symptoms where phenylephrine-related risk is relevant.
- Arrange immediate assessment after suspected overdose, even without symptoms.
- Consider high-anion-gap metabolic acidosis in at-risk patients with deep rapid breathing, drowsiness, nausea or vomiting, particularly with flucloxacillin.
- Guaifenesin may interfere with urinary 5-hydroxyindoleacetic acid or vanillylmandelic acid tests.
Clinical pharmacology
Paracetamol provides central analgesic and antipyretic activity. Oral phenylephrine is predominantly an alpha-adrenergic agonist subject to substantial first-pass metabolism. Guaifenesin reduces sputum viscosity, while cetylpyridinium chloride acts locally as a cationic antiseptic.
Formulation and product differences
- The oral solution contains ethanol, propylene glycol, sorbitol and maltitol.
- It is essentially sodium-free and contains levomenthol, eucalyptus oil and colouring agents.
- A graduated measuring cup is supplied.
