Prescribing considerations
- Confirm that the lesion is suitable for symptomatic self-care; review symptoms persisting beyond seven days and investigate ulcers lasting over three weeks.
- Consider systemic salicylate exposure after excessive or prolonged use because buccal absorption can occur.
- Review concurrent aspirin, salicylates, anticoagulants and uricosurics.
- Seek obstetric input before use during pregnancy.
Contraindications and cautions

- Active peptic ulceration
- Hypersensitivity to salicylates, aspirin, other NSAIDs or product excipients

Monitoring

Clinical pharmacology
Choline salicylate has analgesic and anti-inflammatory activity, principally attributed to inhibition of prostaglandin production. It may be absorbed across buccal mucosa; salicylate is highly protein-bound, metabolised by conjugation and excreted mainly in urine. Cetalkonium chloride provides local bactericidal antiseptic activity.
Formulation and product differences
- Both selected products are clear topical oral gels containing the same active ingredients.
- The original formulation is aniseed flavoured and contains levomenthol and star anise oil; its fragrance components may cause allergic reactions.
- The Cool Mint formulation contains menthol and mint flavour.
- Both formulations contain ethanol and are essentially sodium-free.

Salicylate can be absorbed through the mouth lining. Check with a pharmacist or prescriber when taking medicines affected by salicylates.
Aspirin or other salicylates
Concurrent use may increase total salicylate exposure and should occur only under a doctor’s direction.

Salicylates may increase their effect and potentially increase bleeding risk.
