Prescribing considerations

• Initiate under a physician experienced in metabolic disorders.
• For N-acetylglutamate synthase deficiency, assess individual biochemical responsiveness.
• Review renal function because moderate or severe impairment increases exposure and requires adjustment.
• Coordinate protein restriction or arginine supplementation with the specialist metabolic dietetic plan.

Contraindications and cautions

• Hypersensitivity to carglumic acid or a product excipient.
• Breastfeeding during treatment.

Monitoring

• Plasma ammonia and amino acids, aiming to maintain them within the appropriate reference range.
• Liver and renal function.
• Cardiac function and haematological parameters.
• Clinical and biochemical response during intercurrent illness or metabolic decompensation.

Clinical pharmacology

Carglumic acid is a structural analogue of N-acetylglutamate that activates hepatic carbamoyl phosphate synthetase 1. Oral absorption is incomplete. Elimination is predominantly faecal, with a smaller unchanged urinary component. Exposure rises as renal function declines.

Formulation and product differences

• Both products are scored dispersible tablets intended for dispersion in water; Ucedane is explicitly divisible into four equal portions.
• Carbaglu is supplied in desiccated containers. Before opening it requires refrigeration; after opening it is kept tightly closed, outside the refrigerator and below 30°C, and discarded after three months.
• Ucedane is supplied in aluminium blisters and does not require special storage conditions.
• The products contain different excipients; check product-specific information when allergy, intolerance or switching is relevant.
