Prescribing considerations

• Confirm that the patient can clear loosened secretions through ciliary action and an effective cough.
• Avoid routine combination with antitussives or secretion-drying anticholinergic medicines.
• Review active ingredients, age restrictions and excipients for the specific formulation.
• Consider gastrointestinal bleeding risk, particularly in older people and those taking ulcerogenic or antiplatelet medicines.
• Reassess clinical benefit and continued need if mucus clearance or symptoms do not improve.

Contraindications and cautions

• Hypersensitivity to carbocisteine or a formulation excipient.
• Active peptic ulceration.
• Children under 2 years for selected oral-liquid products.
• Formulation-specific excipient contraindications, including certain hereditary sugar-intolerance disorders.

Monitoring

• Assess change in sputum viscosity, expectoration and respiratory symptoms.
• Ask about dyspepsia and stop treatment for suspected gastrointestinal bleeding.
• Monitor for rash, anaphylaxis or severe cutaneous reactions.
• Consider bronchospasm or secretion retention in patients with asthma, respiratory impairment or a weak cough reflex.
• No routine laboratory monitoring is specified in the selected SmPCs.

Clinical pharmacology

Carbocisteine is S-carboxymethyl-L-cysteine, an orally administered mucolytic that modifies mucus glycoproteins and helps normalise mucus viscosity. It is rapidly absorbed and eliminated mainly through the kidneys as unchanged medicine and metabolites.

Formulation and product differences

• Selected hard capsules contain lactose and are not intended for children.
• Bottled liquids include sugar-containing and sugar-free preparations; excipients may include sucrose, sodium, parabens, maltitol, sorbitol or glycerol.
• The selected sugar-free adult syrup contains alcohol and is not recommended below its product-specific adult age threshold.
• The selected sugar-free sachet is intended only for older adolescents and adults and contains sodium, maltitol, sorbitol and a small amount of alcohol.
• Bottled products are supplied with a measuring device; check formulation restrictions when switching products.
