What is carbidopa + entacapone + levodopa used for?
Licensed use:
Parkinson’s disease in adults with end-of-dose motor fluctuations not stabilised by levodopa plus a dopa-decarboxylase inhibitor.
How does carbidopa + entacapone + levodopa work?
Levodopa enters the brain and is converted to dopamine. Carbidopa reduces its breakdown outside the brain, while entacapone blocks peripheral COMT metabolism, slowing levodopa clearance and extending its response.
How to take carbidopa + entacapone + levodopa
Take exactly as prescribed. Swallow tablets whole; do not split or break them. They may be taken with or without food, although protein-rich meals can reduce levodopa absorption in some people.
Do not change or stop treatment without specialist advice.
Tell the Parkinson’s team if food appears to make symptom control less reliable.
Ask a pharmacist or specialist before crushing tablets or changing formulation.

- Uncontrolled movements or worsening dyskinesia
- Nausea, diarrhoea, vomiting, abdominal discomfort or constipation
- Muscle or joint pain
- Harmless reddish-brown urine
- Dizziness, drowsiness or postural light-headedness
- Hallucinations, confusion, abnormal dreams, anxiety or insomnia
- Reduced appetite or weight loss
- Sweating, rash, tiredness or falls
Get urgent medical advice
- Severe rigidity, fever, confusion or autonomic instability
- Facial or throat swelling, breathing difficulty or another severe allergic reaction
- Persistent diarrhoea, especially with weight loss
- Chest pain, gastrointestinal bleeding or significant breathing difficulty
- Severe muscle pain, weakness or dark urine
- Suicidal thoughts, psychosis, sudden sleep episodes or harmful compulsive behaviour
carbidopa + entacapone + levodopa in pregnancy
Evidence for the combination is inadequate. Product information advises use only when the expected maternal benefit outweighs possible fetal risk. Contact the specialist promptly if pregnant, planning pregnancy or becoming pregnant, and do not stop treatment abruptly.

Product information advises against breastfeeding during treatment. Levodopa enters breast milk and may suppress milk production; infant safety data for the combination are insufficient.
