Prescribing considerations
- Confirm that no concurrent prescription or over-the-counter product contains paracetamol.
- Assess hepatic or renal impairment, alcohol use, low body weight, malnutrition and severe illness before recommending the product.
- Consider total caffeine exposure and susceptibility to insomnia, anxiety or palpitations.
- Persistent or frequent headache warrants clinical review, including consideration of analgesic-overuse headache.
Contraindications and cautions
- Hypersensitivity to paracetamol, caffeine or any excipient.
- Do not use with another paracetamol-containing product.
- The selected product should not be used during pregnancy or breastfeeding.
- Concomitant lithium use is not recommended.
Monitoring
- Routine monitoring is not generally required for occasional labelled use.
- In severe renal impairment, sepsis, malnutrition, chronic alcoholism or other glutathione-depleted states, remain alert for unexplained high-anion-gap metabolic acidosis, especially with flucloxacillin.
- Investigate suspected toxicity urgently after any overdose; early wellbeing does not exclude serious liver injury.
Clinical pharmacology
Paracetamol provides analgesic and antipyretic activity, while caffeine acts as a stimulant analgesic adjunct. Both are readily absorbed orally; the selected tablet’s disintegrant system accelerates dissolution without increasing the total extent of absorption compared with standard paracetamol–caffeine tablets.

Combining products can cause accidental overdose and potentially severe liver injury.
Warfarin and other coumarin anticoagulants
Prolonged regular paracetamol use may increase the anticoagulant effect and bleeding risk.
Lithium
Caffeine can increase lithium clearance, so concomitant use is not recommended.
