Prescribing considerations
- Confirm current campaign eligibility and use an age-appropriate, currently available formulation.
- Review previous COVID-19 vaccination, SARS-CoV-2 infection, allergy history, immunosuppression and planned immune-modifying treatment.
- Postpone vaccination during severe acute febrile illness; minor infection alone is not generally a reason to defer.
- Ensure trained staff and facilities are available to recognise and treat anaphylaxis.
- Record the product name and batch number.
Contraindications and cautions
- Hypersensitivity to bretovameran or any excipient.
- Do not give the same vaccine again after anaphylaxis to a previous Comirnaty vaccination without appropriate specialist assessment.
- Seek specialist allergy advice for suspected polyethylene glycol allergy or a previous systemic reaction to a COVID-19 vaccine.
Monitoring
- Observe after administration according to allergy history and local protocol.
- Explain myocarditis and pericarditis symptoms and when to seek urgent help.
- Use intramuscular-injection precautions with thrombocytopenia, coagulation disorders or anticoagulant treatment.
- Report suspected adverse reactions through the Yellow Card scheme.
Clinical pharmacology
Bretovameran is a single-stranded, capped, nucleoside-modified mRNA encoding the membrane-anchored, prefusion-stabilised SARS-CoV-2 Omicron JN.1 spike protein. Lipid nanoparticles enable cellular uptake and transient antigen expression, eliciting humoral and cellular immunity; conventional pharmacokinetic assessment is not applicable.
Formulation and product differences
- The selected yellow-capped paediatric presentation is a multidose concentrate requiring dilution before intramuscular administration.
- Other presentations are ready to use and intended for different age groups; check cap colour, product name and preparation instructions carefully.
- The selected presentation was marked retired on eMC in July 2026, so current stock may use another variant-targeted formulation.

bretovameran interactions
Bretovameran has few conventional medicine interactions, but treatment and vaccination context can affect administration or immune response.
Immunosuppressants, chemotherapy and immune-modifying treatments
The immune response may be reduced. Coordinate vaccination with the treating specialist and do not stop treatment without specialist advice.
Warfarin and direct oral anticoagulants
These do not directly interact with the vaccine, but injection into a muscle may increase bruising or bleeding. Appropriate precautions are needed.
Other vaccines
Other indicated vaccines may be given at the same visit or separately, using different syringes and injection sites.
