Prescribing considerations
- Confirm that the presentation is consistent with athlete’s foot; seek diagnostic review when uncertain.
- Use caution with previous hypersensitivity to imidazole antifungals such as clotrimazole, econazole or miconazole.
- Consider increased systemic absorption through inflamed or damaged skin, large treatment areas or occlusion, particularly with warfarin.
- Cetostearyl alcohol may cause contact dermatitis; benzyl alcohol may cause allergic reactions or mild local irritation.
Contraindications and cautions
- Hypersensitivity to bifonazole or any excipient.
- Treatment of infant nappy rash.
- Treatment of nail or scalp infection.
- The selected preparation is not for vaginal use.
Monitoring
- Monitor INR appropriately in patients taking warfarin.
- Review the diagnosis if symptoms worsen or fail to improve.
- Assess persistent local irritation or suspected hypersensitivity and discontinue if appropriate.
Clinical pharmacology
Bifonazole inhibits ergosterol biosynthesis at two levels, impairing fungal membrane structure and function. It penetrates infected skin layers. Systemic absorption through intact skin is low but increases with inflammation.
Formulation and product differences
- The selected product is a white cream for external cutaneous use.
- It contains cetostearyl alcohol and benzyl alcohol, which may contribute to local or allergic reactions.
- It is not formulated for vaginal, nail or scalp treatment.

bifonazole interactions
Few interactions are documented for topical bifonazole, but one clinically relevant possibility is recognised.
Warfarin
Topical bifonazole may increase the INR. Closer monitoring may be needed, particularly with occlusion or use over large, broken or inflamed areas.
