Prescribing considerations
- Confirm that wounds are partial thickness and associated with dystrophic or junctional epidermolysis bullosa.
- Evidence is limited in dominant dystrophic and junctional epidermolysis bullosa; regularly reassess benefit.
- Fully assess persistent or worsening symptoms and wound complications.
- Birch-pollen allergy alone does not preclude use because the gel contains no birch-pollen allergens.
Contraindications and cautions
- Hypersensitivity to birch-bark extract or refined sunflower oil.
- Safety and efficacy have not been established below 6 months of age.
- Interrupt treatment if wound infection develops; additional standard treatment may be required.
- Discontinue treatment on any area diagnosed with squamous-cell carcinoma or another skin malignancy.
Monitoring
- Monitor for infection, increased wound size, reopening, bleeding, pain and impaired healing.
- Monitor for local or systemic hypersensitivity.
- Maintain skin-malignancy surveillance appropriate to epidermolysis bullosa.
- Regularly reassess benefit, particularly in dominant dystrophic or junctional disease.
Clinical pharmacology
The triterpene-rich extract modulates inflammatory mediators and activates pathways associated with keratinocyte differentiation and migration. Topical betulin absorption is minimal; the precise clinical wound-healing mechanism is not fully established.
Formulation and product differences
- Sterile, colourless to slightly yellow, opalescent, non-aqueous gel.
- The only listed excipient is refined sunflower oil.
- Each tube is single use and must be discarded after opening.
- For cutaneous use; do not apply to mucous membranes or use ophthalmically.
