Prescribing considerations

• Initiation and supervision should involve a clinician experienced in homocystinuria.
• Use as an adjunct to disorder-specific dietary, vitamin and other treatment rather than as isolated therapy.
• In CBS deficiency, rising methionine requires review of biochemical control, dietary adherence and treatment.

Contraindications and cautions

• Hypersensitivity to betaine anhydrous.
• Use particular caution in CBS deficiency because treatment-related hypermethioninaemia has been associated with cerebral oedema.

Monitoring

• Monitor plasma total homocysteine to assess biochemical response.
• Monitor plasma methionine, particularly in CBS deficiency and whenever headache, vomiting or visual symptoms develop.
• If methionine rises, assess dietary adherence and increase monitoring as clinically appropriate.

Clinical pharmacology

Betaine is a methyl donor for remethylation of homocysteine to methionine. Homocysteine lowering begins within several days, while methionine and S-adenosylmethionine may increase. Betaine concentrations do not reliably indicate treatment efficacy.

Formulation and product differences
