Prescribing considerations
- Confirm the indication and, for syphilis, disease stage and possible neurological involvement.
- Check previous immediate reactions to penicillins and other beta-lactams, asthma and allergic diathesis.
- Assess renal and hepatic function and avoid confusion with benzylpenicillin sodium or procaine benzylpenicillin.
- Use meticulous deep-intramuscular technique; aspirate before injection and stop if blood is aspirated or marked pain occurs.
- Prepare for immediate management of anaphylaxis and counsel patients treated for syphilis about the Jarisch–Herxheimer reaction.
Contraindications and cautions
- Hypersensitivity to penicillins or any product excipient
- Previous severe immediate hypersensitivity reaction to another beta-lactam antibiotic
- Contraindication to lidocaine when lidocaine is selected as the diluent
Monitoring
- Observe after administration when allergy risk or clinical circumstances warrant it.
- During prolonged or repeated treatment, periodically assess renal, hepatic and haematological function.
- Monitor for superinfection, candidiasis and antibiotic-associated colitis.
- Monitor INR closely with an oral vitamin K antagonist.
- For syphilis in pregnancy, ensure specialist treatment documentation and neonatal follow-up planning.
Clinical pharmacology
The benzathine salt forms an intramuscular depot that is slowly absorbed and hydrolysed to active benzylpenicillin. It inhibits bacterial cell-wall synthesis, has time-dependent activity and is eliminated mainly through the kidneys. Cerebrospinal-fluid penetration is inadequate for neurosyphilis.
Formulation and product differences
- The selected product is a single-use powder reconstituted to form a particulate intramuscular suspension.
- Water for injections or lidocaine may be compatible diluents; lidocaine can reduce injection-site pain but requires its own contraindication checks.
- The suspension should be homogeneous and used promptly. Needle blockage can occur, so follow product-specific preparation instructions and administer slowly and steadily.
- This depot, intramuscular-only salt must not be substituted for short-acting benzylpenicillin sodium.

Methotrexate
Penicillin may reduce methotrexate elimination and increase toxicity; concurrent use is not recommended by the product information.
Oral vitamin K antagonist anticoagulants
The anticoagulant effect and bleeding risk may increase, so closer INR monitoring may be required during and after treatment.
Probenecid
It reduces renal secretion of benzylpenicillin, increasing and prolonging blood concentrations.
