Prescribing considerations

• Confirm that the lesion is superficial; severe burns, deep wounds or suspected infection require assessment.
• Avoid the eyes and mucous membranes.
• Explain the fire risk from contaminated clothing, bedding and dressings.
• Sodium benzoate may cause local irritation and may increase jaundice in babies up to 4 weeks old.

Contraindications and cautions

• Hypersensitivity to an active ingredient or excipient.
• Consider previous sensitivity to lanolin, fragrances, balsam-related compounds or other components.

Monitoring

• No product-specific laboratory monitoring is described.
• Review for hypersensitivity, worsening inflammation, infection or failure to improve.
• In newborns, consider the sodium-benzoate warning and assess jaundice where clinically relevant.

Clinical pharmacology

Zinc oxide is protective, soothing and astringent. Benzyl alcohol has local anaesthetic and disinfectant properties. Benzyl benzoate is acaricidal, benzyl cinnamate contributes mild antiseptic activity, and lanolin supports the barrier base.

Formulation and product differences

• The product is an emulsified water-in-oil cream.
• Excipients include sodium benzoate, paraffin waxes, liquid paraffin, synthetic beeswax, propylene glycol, BHA and fragrance components including lavender.
• Paraffin-containing residue on fabric creates a serious fire hazard; washing reduces but may not completely remove build-up.
